Cynics have long questioned the validity of epidemiological research that demonstrates a direct inverse relationship between increasing physical activity (PA) and morbidity preponderance or all-cause mortality. Furthermore, recommendations i.e. 150 minutes per week of moderate-to-vigorous PA, derived from such research is criticised as purely guesswork. It is proposed that maybe this relationship is a case of reverse causality where it is not so much that higher PA levels concomitantly lead to better health but rather that better health leads to higher PA levels. PA levels are therefore a consequence (and symbolic) of, but not causative of good long-term health. Very long-term and controlled intervention studies are difficult to perform and are lacking which requires that other methods be used to gain insight into what impact PA has on health and ageing. 
That is why this study (see picture) that explores twins that differ with respect to leisure-time PA throughout their adult life is so interesting. Results revealed that when monozygotic (identical) twins were compared, differences in body fat, structure of heart, arteries, bones and in metabolic and regulatory pathways were apparent. The authors concluded that these identified differences “may partly explain the mechanistic links between long-term PA, phenotypic changes, and decreased risk of cardiometabolic diseases.“ Basically, this type of evidence supports what epidemiological research has been telling us for over 30 years now, and that is, participation in regular PA positively influences health over the long-term and reverse causality does not fully explain this relationship. Perhaps most importantly from a public health perspective is that the most inactive who then start to consistently and modestly increase their physical activity levels (10-20 minutes per day) realise substantial health benefits.
